FAME Program Policies Agreement

Before signing, make sure to review the Studio Policies and Terms of Service. Check the boxes to indicate that you read and understand the terms, conditions, and studio policies, then sign and submit to proceed to checkout. By signing, you agree to these terms, conditions, and studio policies.

I understand that there are no refunds or stop payments for EMH/FAME PA programs and services. By registering, I agree to pay tuition in full or utilize EMH/FAME PA’s payment plan with the payment information saved to my account profile. Even if I have to unenroll for any reason during any point in the semester, I am still financially committed to EMH/FAME PA until balance is paid in full.

EMH/FAME PA are in no way responsible or liable for any injuries/accidents that occur on site.

I understand all EMH/FAME PA studio policies and procedures including but not limited to hours of operation, lesson make-up policy, attendance, canceling lessons/programs, and payments/billing including refunds.View Studio Policies

Encore Music Hall and/or FAME Performing Arts, referred to as EMH/FAME PA hereafter, sets the following terms of service:

I understand that if I need to cancel or stop any EMH/FAME PA programs, a written notice must be given to the front office by the last business day of the month by 5pm.

Medicine Policy: EMH/FAME PA are not authorized to administer medicine (unless life altering medications such as Epi-Pens, anti-seizure medication, etc.) unless a current doctor’s note with the medication name, expiration date, dosage amount/time, and child’s name on the note and medication label. This includes over the counter medication such as Tylenol, Ibuprofen, and cough/cold medication.

Sick Policy: if your child is experiencing any kind of symptoms, please keep them home 24 hours until symptoms reside without the aid of medication. If they are experiencing symptoms at EMH/FAME PA, parent/guardian or emergency contact will be notified to please come pick up immediately.

Allergies: if your child has allergies and needs medication (Epi-Pen or any other type of medication such as Benadryl), the same guidelines stated in our Medicine Policy must be followed. Allergies must be stated and the signs/symptoms (rash, difficulty breathing, etc.) with the allergy action plan (first give Benadryl at the sign of symptoms or immediately administer Epi-Pen, etc.) to follow the first sign of ingestion/contact or symptoms must be clearly stated in the doctor’s note.

I understand that by registering for a Troupe program I have committed to EMH/FAME PA’s production regardless of how my child is cast. I understand that all Troupe rehearsals are required. If my child misses too many rehearsals for any reason I understand it will be at the discretion of the director to recast my child’s role. I understand I must be present for the entire time for Tech Week, or my child may be recast.

I understand that for any lost or misplaced scripts, the replacement fee is $25 per script. Payable by check to FAME Performing Arts or by payment information saved to my account profile.
